Funding available for community-based organizations (CBOs) serving:<\/span><\/strong><\/span><\/p>\n<ol>\n<li><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\">Expectant mothers and parents of children ages 0-4<\/span><\/li>\n<li><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\">Out of school, out of work young adults ages 16-24 (sometimes referred to as \u201cdisconnected youth\u201d or \u201copportunity youth\u201d)<\/span><\/li>\n<li><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\">Unemployed or underemployed working age adults ages 18 and over<\/span><\/li>\n<\/ol>\n<p><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\">The Mayor\u2019s Fund to Advance NYC, with the assistance of CEO and the Department of Health and Mental Hygiene, are seeking organizations for the Connections to Care (C2C) program, supported by the federal Social Innovation Fund. The program will evaluate how increasing capacity to provide mental health services can affect outcomes on indicators related to employment, education, and early-years development.\u00a0 Proposals are due on October 19, 2015.\u00a0<\/span><\/p>\n<p><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\"><strong><em>How could C2C help my organization?<\/em><\/strong><br \/>\nStaff at organizations serving low-income communities often cite a need for increased mental health services, noting that mental health conditions prevent program participants from achieving their program goals.\u00a0 Through C2C, your organization could receive funding to form a partnership with a licensed Mental Health Provider (MHP).\u00a0 The MHP and your organization will create a plan for training your staff in non-clinical yet effective techniques for addressing common mental health conditions.\u00a0 Envisioned as a five-year program, C2C will also provide funds for the MHP to support your staff with ongoing coaching and referral partnerships.\u00a0<\/span><\/p>\n<p><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\"><strong><em>How can I identify a Mental Health Provider to partner with?<\/em><\/strong><br \/>\nReach out to hospitals, clinics, or practitioners in your community to discuss this opportunity.\u00a0 Some MHP\u2019s seeking a CBO partner have listed their contact information on the Mayor\u2019s Fund website and their information can be found <a href=\"http:\/\/www.nyc.gov\/html\/fund\/downloads\/pdf\/MHP_Seeking_CBO_Partner_for_C2C.pdf\" target=\"_blank\" rel=\"noreferrer\"><strong>here<\/strong><\/a>.<\/span><\/p>\n<p><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\"><strong><em>How can I apply?<\/em><\/strong><br \/>\nThe Request for Proposals can be accessed <a href=\"http:\/\/www.nyc.gov\/html\/fund\/html\/projects\/connections-to-care.shtml\" target=\"_blank\" rel=\"noreferrer\"><strong>here<\/strong><\/a>.<\/span><\/p>\n<p><span style=\"font-family: Verdana, Arial, Helvetica, sans-serif; font-size: small;\">Do you have additional questions?
